About

Sunrise began in 1973 as a migrant health clinic. Today, our family medicine, pediatric, prenatal, dental, and school-based clinics support the health of nearly 42,000 in Northern Colorado.We are a recognized leader in safety net services, health information technology, community collaborations, and healthcare professional education.

​Sunrise Community Health is a private non-profit 501(c)(3), supported in part by Health Center Program funding from the Bureau of Primary Health Care.

Financial Assistance

We serve everyone, regardless of ability to pay

Sunrise accepts CHP+, CICP, Health First Colorado (Medicaid), Medicare, private insurance, and payment plans.

Health First Colorado (Medicaid) is a public health insurance program for Colorado residents who qualify.

Connect for Colorado can help you apply for federal financial assistance to purchase affordable health insurance via the Health Insurance Exchange.

Colorado Health Plan Plus (CHP+) is a public health insurance program for pregnant women and children who qualify.

Sunrise programs include a sliding fee discount, pharmacy assistance, a prenatal care discount, and women’s health programs.

Colorado Indigent Care Program (CICP) is not a health insurance program, but supports discounted health services to low income individuals and families not eligible for Medicaid or CHP+. Hospitals and clinics throughout Colorado participate in CICP.
