SUNRISE
Sunrise Community Health is a non-profit, award-winning healthcare provider serving northern Colorado from twelve clinic sites in Greeley, Evans, and Loveland, Colorado.
Sunrise is a Federally Qualified Health Center (FQHC). As an FQHC, Sunrise Community Health is eligible for Federal Torts Claim Act (FTCA) medical malpractice liability protection. As such, this health center receives Human and Human Services (HHS) funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Affordable: We ensure that care that is available to all, without regard for ability to pay or insurance status. We offer a sliding fee scale and can help you enroll in Health First Colorado (Medicaid).
Primary Care: We deliver integrated primary care services for all ages, including medical, behavioral health, vision, and dental care, as well as on-site laboratory, X-ray, pharmacy, health education, interpretation, and care management services.
Team-based strategy: A team of health care professionals work together to support your care and may include a primary care provider, behavioral health professional, dentist, dental hygienist, registered nurse, pharmacist, medical and dental assistant, social worker, health educator, and more.
Support: We offer support services and connect you to community resources to address barriers in accessing care.

of our patients and our community; more than 50% of the
seats are always filled by patients who utilize our services.
